Coordination of Services

Coordination of Services means that because your loved one may need a wide variety of services, you also need someone to make sure all of these services are carefully selected and properly coordinated based on insurance coverage and financial needs.

 

Mobility - Are they less mobile? Have you noticed more falls and/or bruises? Do they need to hold on to the wall or other objects when moving about?

 

Isolation - Have they become less social? (aside from the pandemic) Are they more withdrawn? Have they been sick more often? Have they been communicating less? Have they stopped or altered their daily routines?

 

Dietary - Have they lost interest in food and are drinking less water (a frequent precursor to a urinary tract infection, especially for women).

 

Stress management - Are they suddenly acting differently than they normally do? Is there a change in their personality, such as being quieter, more aggressive, or more flamboyant than usual?

 

Technology - Are they struggling with using the phone, TV or microwave? Do they become easily frustrated and lose their temper with friends and family?

 

Scam Calls, Elder Abuse - Have they sent money to unusual causes? Is their credit card balance higher than normal? Is there a new “friend” in their life who may be visiting or calling a little too often?

 

Finances and Paying Bills - Are any bills overdue? Is your loved one behind on any of their bills? Have any utilities been turned off due to non-payment? Have they received any late notices?

 

Skilled Nursing Home Care - Do you have a loved one who needs 24/7 support care? We help to select services and individual caretakers that are the best match for your loved one.

 

Caregiver – Care includes hiring an individual from a local home care service. They provide personal care for your loved one to enable more independent living. The personal care provider can include, but is not limited to, light housekeeping, making meals, bathing, and ensuring your loved one is taking his or her meds. We coordinate client needs with medical professionals.

 

Medication and Medical Management – Does your loved one need prescriptions delivered or picked up? Could they use help to ensure they do not run out of prescriptions? Are they taking their medications correctly? Are expired medications being disposed of properly?

Property & Auto Sales – Whether family is nearby or not, it can be helpful to have a neutral party provide assistance in arranging the sale of a home and/or vehicles. We provide guidance to determine whether a property or a vehicle needs to be sold or can be modified to support quality and enjoyment of life for your family member without having to move. If it is best that a home or property be sold, we work with local realtors or the professional realtor of your choice to help with the sale of the home and to support transitioning from the home to another residence.

 

Pet Care – Pets can be a vital part of helping a family member to feel happier, less stressed and more at ease when things are changing around them. We can arrange support services such as pet grooming and veterinarian appointments to help ease the burden of caring for pets.

 

Wellness & Fitness – Daily exercise and attention to diet are key to quality and longevity of life. If your loved one needs assistance in this area, we provide guidance and support in selecting the right mix of services to serve your family member.

 

Other Services – We arrange lawn care and home inspections to guarantee regular replacements of filters, and asbestos and smoke alarm batteries. Setting barber shop and hair salon appointments are examples of services we provide so your family member is living in a safe home that is well cared for as well as getting out socially for self-care appointments and meeting friends.
